In 2014 the Family Health Center received grant funding to reconfigure 2,700 sf of their existing space to accommodate its Patient Centered Medical Home (PCMH) care delivery model. The project consolidated four separate nurses’ stations into two larger nurses’ stations, created a space to accommodate multi-disciplinary team huddles, integrated behavioral health counseling and improved patient flow as well as patient privacy.
